NVMA Health & Wellness Plan
NVMA Offers Blue Cross Blue Shield Health Insurance – The NVMA strives to provide our members with benefits that work for you. With that in mind we, are pleased to once again offer a group health and wellness plan with Blue Cross and Blue Shield of Kansas.
Current members of the NVMA, and your staff, are eligible to participate in the BCBS Group Health Insurance Plan.
Current Plan Participants – please review your plan. Changes may be made during the open enrollment period. A complete change form must be submitted to Bukaty Companies.
New Enrollees – a completed BCBS enrollment form must be submitted to Bukaty Companies. There is no health questionnaires required. Forms may be emailed or faxed to Bukaty Companies to ensure applications are received in time for processing.
IMPORTANT: New enrollment forms or change forms must be completed in full and returned to Bukaty Companies by September 18, 2026.

*Due to Federal guidelines, eligibility in the NVMA health and wellness plan has changed. To be eligible, you must be an employer. This means that you must have at least one other person on your payroll in a full or part-time position and your employees must be offered the opportunity to enroll in the plan, regardless of whether they actually choose to enroll.
